How to File a Complaint
The mission of the Executive Council is to protect the health of those in Texas who find themselves in need of physical therapy or occupational therapy services. To that end, the agency investigates all complaints made against licensees or facilities registered by the PT and OT Boards. If you have a complaint against a licensee or registered facility of either Board, you are encouraged to notify this agency by one of the following methods:
- Complete the Investigation Referral Form online.
- Download, complete and mail or fax to us the Investigation Referral Form at the top of this page.
- You may also call the Health Professions Council hotline (1/800-821-3205 - complaints only) or this agency directly at 512/305-6900.
